Policy and Rules
- The membership charge is auto-drafted.
- A minimum of a 3-month subscription is required.
- No fees for cancellation after 3 months, but you must give a 15-day notice to cancel by signing a consent form provided by Face 5, LLC.
- Membership, upgrades, and discounts cannot be gifted or shared.
- One treatment credit is $49
- Each treatment credit expires 3 months after the monthly charge, even if the membership has been cancelled.
- Unused treatment credit can not be gifted to other clients.
- You will still receive 10% off services and products for 3 months after cancellation.
- Only 1 treatment credit can be used per visit. Treatment credits cannot be stacked.
- You may pause the membership for up to 3 months for health reasons or deployment.
- Must be an active member during your birthday month to receive your retail birthday credit. Birthday Credit can not be gifted. Birthday credit must be used during the birth month.
- Membership discounts may not always be combined with other offers. Monthly memberships may not be cancelled during the first 3 months of subscription. No refunds will be issued on monthly payments.
