The membership charge is auto-drafted.
A minimum of a 3-month subscription is required.
No fees for cancellation after 3 months, but you must give a 15-day notice to cancel by signing a consent form provided by Face 5, LLC.
Membership, upgrades, and discounts cannot be gifted or shared.
One Credit is $49
May save up to 3 club credits to use later.
Unused credit can not be gifted.
Credit will be forfeited 90 days after the date of auto-draft.
Unused saved treatments expire 30 days after cancellation or completion of membership.
Only 1 credit can be used for a single treatment. Unused credit cannot be combined.
You may pause the membership for up to 3 months for health reasons or deployment.
Must be an active member during the birth month to receive birthday credit – Birthday Credit can not be gifted. Birthday credit must be used during the birth month.
Memberships may not always be combined with other offers. Monthly memberships may not be canceled during the first 3 months of subscription. No refunds will be issued on monthly payments.
